The SouthEnd MHA Executive is seeking your feedback on your son or daughter's experiences in the 2013-14 season, as we finalize our planning for the upcoming 2014-15 season. We are using Survey Monkey to collect completely anonymous and confidential feedback from you. Just click on the link below and complete the form - it will take only five minutes of your time and will provide us with the feedback we need to ensure this coming season is as fun and filled with development for your child as possible.
If more than one child from your family participates in SouthEnd hockey programs, you can complete the survey for each child. Again, all feedback is completely confidential and anonymous. The survey will run until mid-August.
